Schools, sports leagues, employers, and camps each have their own form — and their own deadline. Bring the paperwork and we'll complete it during your visit, including California CIF pre-participation forms for student athletes. The exam covers history, vitals, vision, heart and lung check, and musculoskeletal screening, with vaccines updated if needed.
